Medications to be taken during the school day should be brought to the school nurse by a parent or designated adult. Physician Authorization in writing or fax is required for all medications (over the counter and prescription). The Alcott Health Office fax number is 978.341.2459. Parent/Guardian permission in writing is also required. The Parent/Guardian Medication Authorization Form is available at the Health Office or on the School Health Forms page. The medical order form must be renewed at the beginning of each school year and whenever there is a change in the dose or a discontinuation of the medication.
1. Over-the-counter medications
These medications include cough/cold remedies, throat lozenges, pain relievers such as Ibuprofen (Advil, Motrin) and medicated topical applications. All medication must be in manufacturer-labeled containers.
2. Prescription medications
Prescription medications must be in a pharmacy labeled bottle (ask the pharmacist to dispense a duplicate bottle for use at school). The pharmaceutical container must state the student's name, the name of the medication, the dose and frequency to be administered, and the name of the prescribing physician. For short-term antibiotics, given for less than two weeks, a medical order is not necessary; the parent signature and the prescription labeled container is sufficient authorization.
Acetaminophen (Tylenol) or Ibuprofen (Motrin) can be administered to your child by the school nurse if you indicate your permission on the Student Health/Emergency Information Form that parents submit at the beginning of each school year. This form is available on the Parent Portal page. The school nurse follows school district protocol when assessing and administering this medication.
