School Entry Requirements

There are health requirements that all students must meet prior to admission to school. They are the Massachusetts mandated immunization requirements and a current physical examination. Students entering kindergarten must additionally submit the date of a lead screening.

Immunization Requirements

The Massachusetts Department of Public Health requires that all school children be properly immunized. All students who are new to Massachusetts's schools must also comply with this requirement. The only exceptions that can be made are for religious or medical reasons. If a religious exemption is requested, then a signed statement by the parent/guardian must be on file in the health record; if the exemption is required on medical grounds, a signed statement from the doctor must be on file in the health record. An Immunization Exemption Form is available on the School Health Forms page. The school nurse will notify parents/guardians of students with incomplete records. Immunizations should be completed prior to school entry. Incomplete immunizations may result in exclusion from school until the requirements are met.

The Massachusetts Department of Health has established the following immunization requirements for school entry:

  1. Measles Vaccine (MMR)
    All students entering elementary school are required to have two (2) doses of the MMR vaccine.
  2. Diphtheria, Tetanus, Pertussis Vaccine (DPT)
    All students entering elementary school are required to have 5 doses of DPT vaccine unless the 4th dose was given after the 4th birthday, then only 4 doses are required.
  3. Polio Vaccine
    All students entering elementary school are required to have 4 doses of polio vaccine unless the 3rd dose was given after the 4th birthday, then only 3 doses are required.
  4. Hepatitis B Vaccine
    All students entering elementary school are required to have 3 doses of hepatitis B vaccine.
  5. Varicella
    All students entering elementary school must have two doses of Varicella vaccine OR a physician-certified reliable history of the chicken-pox disease. This history must include the month and year of the disease.

Lead Screening

Students entering kindergarten must present evidence of having been previously screened for lead poisoning.

Physical Examinations

The Massachusetts Department of Public Health requires physical examinations of students during the year of entrance into the public school system, or within six months before entrance. A copy of the physical exam is required on file in the Health Office. A student transferred from another school system shall be treated as an entering student unless health records are transferred showing that there has been adequate health appraisal in the school of transfer.
